multiple label menu with text area and button using GUIs in python

Using the code below, I need to create a multiple label menu containing each line. The text area on the window and button asks the user to enter the number of their choice. The showWarning dialogue then displays the message example "You selected to enter a new item."



I tried a couple of methods of GUI but was unsuccessful. It would be great if somebody can help me with this.
